Pure Identity Membership Agreement

Effective Date: April 2026

By enrolling in any "Pure Identity" membership tier at Pure MedSpa, the Client agrees to the following terms, billing policies, service limitations, and account conditions. This Agreement is binding on the Client and governs membership use, recurring payments, included benefits, cancellations, and early termination.

Membership Rules — Read First

  • Monthly auto-billing required
  • 6 or 12-month commitment depending on tier
  • Benefits apply only while membership remains active and in good standing
  • Unused benefits, credits, units, and discounts are forfeited upon cancellation or termination
  • Early cancellation before the minimum term triggers retroactive pricing adjustment under the clawback policy

1Commitment, Billing & Auto-Renewal

  • Memberships require a minimum 6 or 12-month commitment depending on tier.
  • Membership fees are charged automatically every 30 days to the payment method on file.
  • By enrolling, the Client authorizes Pure MedSpa to process recurring membership charges and any authorized balances due under this Agreement.
  • After the initial minimum term is completed, the membership converts to a month-to-month arrangement unless cancelled in accordance with this Agreement.
  • All membership fees, setup fees, and charges previously processed are non-refundable, except where required by law.

Payment Failure

If a payment is declined or fails for any reason, Pure MedSpa may immediately suspend membership benefits, Member pricing, included units, banked credits, discounts, and booking access until the account is brought current. If the unpaid balance is not resolved, the membership may be terminated and any remaining amount owed may still be charged to the payment method on file or otherwise pursued as permitted by law.

2Credits, Units & Membership Value

  • Membership credits, banked balances, included services, toxin units, discounts, and promotional benefits have no cash value and may not be redeemed for cash, refunded, or exchanged.
  • Membership benefits are non-transferable and may only be used by the enrolled Client.
  • Benefits remain available only while the account is active and in good standing.
  • Rollovers apply only to tiers that expressly include a banked balance or treatment credit. Not all memberships include rollover credits or rollover units.
  • Any unused credits, units, discounts, promotional benefits, and banked value are forfeited immediately upon the effective end of membership.

3Tier-Specific Terms

The Pure Bank ($50/mo)

12-Month Commitment

100% of the monthly fee is banked as account credit. Includes Member pricing for neurotoxins and 10% off skincare. Banked credits roll over only while the membership remains active and in good standing.

The Pure Tox ($149/mo)

12-Month Commitment

Requires $199 Non-Refundable Setup Fee

Provides up to 40 units of Botox or 120 units of Dysport every four (4) months, subject to medical appropriateness and provider assessment. Unused units do not roll over beyond the applicable treatment cycle.

The Pure Membership ($199/mo)

6-Month Commitment

Provides up to $400 in monthly treatment credit for eligible Skin and Laser services only. Credits may not be used for injectables. Unused eligible credits roll over only while the membership remains active and in good standing.

The Pure Elite ($387/mo)

12-Month Commitment

Requires $199 Non-Refundable Setup Fee

Includes Unlimited Face Toxevery four (4) months, measured from the Client's last Elite tox visit, plus $300 in monthly credit for any laser or facial service. Monthly credit may not be used for injectables or add-ons. All benefits remain subject to medical appropriateness, provider evaluation, and standard clinical dosing limits.

Elite Tox Scope of Service

Included in Standard Elite Coverage:

  • Forehead (Frontalis)
  • Frown Lines / 11s (Glabella Complex)
  • Crow's Feet (Orbicularis Oculi)
  • Lip Flip (Orbicularis Oris)
  • Chin (Mentalis)
  • Bunny Lines (Nasalis)

Touch-Up Policy: One (1) complimentary touch-up visit is allowed within 2 weeks of an Elite tox treatment, when clinically appropriate and approved by the provider.

Clinical Limit: Standard clinical dosing and provider judgment always apply. Requests beyond safe clinical norms may be denied, modified, or require additional charges.

Additional Tox Before the Next Included 4-Month Visit (Billed at the $10/unit Add-On Rate):

  • If four (4) months have not passed since the Client's last Elite tox visit, any additional tox requested is billed at $10/unit.
  • Masseters (Jaw Slimming / TMJ)
  • DAO
  • Neck / Platysmal Bands (Nefertiti Lift)
  • Hyperhidrosis (Underarms)
  • Gummy Smile
  • Traps Tox

4Clinical Evaluation & Service Limitations

  • All services are subject to clinical evaluation, treatment suitability, provider judgment, contraindications, and medical appropriateness.
  • Pure MedSpa may refuse, reschedule, limit, or modify treatment for safety, medical, or operational reasons.
  • Membership enrollment does not guarantee that any specific treatment will be performed.
  • Treatment outcomes vary by person, and results are not guaranteed.
  • Memberships govern access to pricing and benefits, but all medical and aesthetic services remain subject to provider discretion and standard of care.

5Freeze & Cancellation Rules

Account Freezing

Available only for The Pure Membership ($199/mo), unless otherwise approved in writing.

  • Notice:At least 7 days' notice is required before the next billing date.
  • Limit: One (1) freeze of up to 30 days per contract year.
  • Extension: The minimum commitment period will be extended by the total length of the approved freeze.
  • Restriction: During a freeze, credits, booking privileges, and membership benefits may be paused or restricted.
Cancellation Process
  • Notice: A 30-day written notice is required by SMS, phone, email, or another approved written method.
  • Final Billing: If a billing date occurs during the notice period, that charge remains due.
  • Effective End Date: Cancellation becomes effective at the end of the final active billing cycle.
  • Forfeiture: All unused credits, units, discounts, and benefits are forfeited at the effective end of membership.

6Early Termination Clawback

If the Client cancels before completing the applicable 6 or 12-month minimum commitment period, Pure MedSpa may retroactively bill the difference between the discounted Member pricing received and the Standard Retail pricing for all services, treatments, units, discounts, and benefits provided during the membership.

The Client authorizes Pure MedSpa to charge this balance to the payment method on file. If the payment method cannot be successfully charged, the Client remains responsible for immediate payment of the balance due.

Example

If a Client received Botox at a discounted Member rate of $12/unit but the standard retail price was $14/unit, the Client may be billed the $2/unit difference for all applicable units received before the required commitment period was completed.

7Refunds, Chargebacks & Account Enforcement

  • Membership fees, setup fees, used benefits, applied discounts, and consumed services are non-refundable except where required by law.
  • A payment dispute or chargeback does not cancel the Client's contractual obligations under this Agreement.
  • Pure MedSpa reserves the right to suspend services, revoke benefits, and recover any unpaid balance, clawback amount, or improperly disputed charge to the fullest extent permitted by law.

8Legal Terms

  • To the maximum extent permitted by law, Pure MedSpa shall not be liable for indirect, incidental, special, or consequential damages arising out of membership participation or services rendered.
  • If any provision of this Agreement is found unenforceable, the remaining provisions will remain in full force and effect.
  • This Agreement shall be governed by and interpreted under the laws of the State of Washington.

Digital Agreement & Authorization

By processing your setup fee, initialization payment, or first monthly membership charge through Pure MedSpa, you confirm that you have read, understood, and agreed to these Membership Terms. You specifically authorize recurring monthly charges to your payment method on file and acknowledge the applicable minimum commitment period, cancellation notice requirement, forfeiture rules, non-refundable charges, and early termination clawback policy.
